To determine your correct size, measure just above your bust, Small 30"-35", Medium 34"-37", and Large is 38"-42". If your have a newborn you may need to drop an inch or so off of your above the bust chest measurement to compensate for expected weight loss. Carrier size generally corresponds to your normal pre-pregnancy fitted t-shirt size.
